Monday through Friday between 8:00 a.m. and 5:00 p.m. (Up
to 29 hours per week)
ESSENTIAL
JOB FUNCTIONS
•
Under the direction of the CSS Transportation Coordinator,
responsible for the operation of the New Freedom Transportation
Voucher Program for 200 clients at Cobb Senior Services.
• Recruit new vendors and monitor compliance of transportation
vendors used. Ensure that clients are provided an updated
list.
• Assist clients with completing Para-transit application
forms within required time. Contact and follow-up with physician's
offices to effect completion of needed medical forms. Maintain
on-going contact with Para-Transit to determine results
of applications.
• Remove clients from waiting list; receive referrals from
CCT Para-transit and/or the general public. Complete in-home
or office assessments, determine eligibility, maintain &
monitor program compliance. Make referrals to Case Management
when needed.
• Advertise and market new program through community contacts,
flyers, etc.
• Maintain documentation as directed and record all information
and data in computerized client assessment and tracking
system (ServTracker).
• Disburse Vouchers, track client spending and compliance,
keep track of expiration dates. Disburse additional vouchers
as needed.
• Receive spent vouchers from vendors, review and determine
validity of expense, record on spreadsheet and in ServTracker,
forward to DOT for payment. Submit monthly service reports.
• Perform other related duties as assigned
MINIMUM
QUALIFICATIONS
•
High School Diploma
• Some knowledge of aging, case management, geriatrics and
transportation
• Experience working with computers and entering data in
spreadsheets
• Experience working with the elderly desirable
• 1-2 years experience in a related field
